Class
Electrolyte
MOA
Increases cardiac contractility and may enhance ventricular automaticity. Stabilized cardiac cell membranes in hyperkalemia.
Indications
Calcium channel-blocker OD (first line), beta blocker OD (second line), cardiac arrest, hypocalcemia, hyperkalemia.
Contraindications
Pts taking Digoxin, leads to digoxin toxicity.
Adverse reactions
Tingling sensation, nausea/vomiting, dry mouth, increased thirst.
Drug interactions
May worse arrhythmias secondary to digitalis toxicity. Do not mix with or infuse immediately before or after sodium bicarbonate without intervening flushing.
Dosage
Adult:
Hyperkalemia: 0.5-1g slow IV/IO push
Beta or calcium channel blocker OD: 3-6g IV/IO
Duration of action
Onset: 5-15 mins
Peak: 3-5 mins
Duration: 15-30 mins. May go to 4 hrs depending on dose.
Special considerations
Does not irritate the vessels as much as calcium chloride, it is a slightly weaker concentration.
