S/s similar to space occupying lesion
But progress more rapidly
Uncommon disorder
Risk
Compromised immune:
Systemic illness
Steroid use
Immunosuppressants
Cytotoxic chemo
Predominantly occur
Parietal lobe from blood borne infections
Otitis and mastoiditis
Temporal lobe and cerebellum
Sinusitis
Frontal and temporal lobes
Days 1-3
Involvement of cerebrum
Inflammatory infiltrates w/in 24 hours
By 3, increased perivascular inflammation
Days 4-9
Center is necrotic
10-13
Early astrocytes surround infection and capsule forms
14 days - 5 weeks
Necrotic center shrinks, capsule thickens, astrocyte scarring
S/S
HA recent onset
High WBC
Expanding intracranial mass
Possible herniation and BS compress
Stiff neck rare, normal body temp common
EEG

Tx
Antibiotic
Surgical drainage
Monitor CT
Corticosteroids initially for edema
Antibiotics and surgery
Lower mortality from 65% to 30%
Neurological sequalae
50%
