SALTR-Harris classification of fractures

A

I. SLIPPED; the fracture plan poses all the way through the growth plate not involving bone and prognosis is good
II. Above; fracture passes across most of plate and up through the metaphysis, prognosis is good. MOST COMMON
III. LOWER: fracture passes some distance along g.p. and down through epiphysis. Prognosis poorer as proliferative and reserve zones are interrupted.
IV: THROUGH/TRANSVERSE: fracture passes directly through metaphysis, gp and down epiphysis. Poor prognosis.
V: Ruined: Crushing type injury that does not displace gp but damages it by direct compression. Worst prognosis. required anatomic reduction.

septic arthritis cause and tx

A

Septic arthritis in children is most commonly caused by S. aureus, and is the most important diagnosis to rule out in children presenting with unilateral knee or hip pain or limp, in addition to redness, warmth, swelling, and severe pain of the affected joint.

Joint aspiration is the only completely reliable method for ruling out the diagnosis. Treatment involves hospitalization, appropriate antibiotic coverage (generally IV x7 days, 3 to 4 weeks total), and surgical irrigation.

Developmental dysplasia of the hip (DDH)

A

DDH represents a spectrum of anatomic abnormalities in which the femoral head and the acetabulum are in improper alignment and/or grow abnormally. These include dysplastic, subluxated, dislocatable, or dislocated hips. Some 60% to 80% of abnormal hips of newborns identified by physical examination resolve spontaneously by 2 to 8 weeks of life. Ultrasound not warranted at first.
