What is gold standard and why is it not used?
Gold standard is isolation of virus in tissue culture. Difficult and not clinically useful to make treatment.
What test is used for diagnosis?
RT-PCR because it is rapid and has high sensitivity and specificity.
SSRNA enveloped virus
- Paramyxoviridae family
- Four major serotypes (1-4)
- Reinfection is common
Parainfluenza
What type of cell does Parainfluenza preferentially affect?
Ciliated epithelial cells
How does immune response drive Parainfluenza effects?
- Virus enters
- Neutralizing antibody builds up
- Mucosal IgA tries to attack virus
- Inflammatory response
Which type of airway does Parainfluenza affect?
Large airways of lower respiratory tract.
Causes croup, bronchiolitis
What can Parainfluenza cause in immunosuppressed patients?
Pneumonia. 37% Mortality.
How is croup or bronchiolitis treated?
Supportive Care
How are immunosuppressed patients treated?
Reduce immunosuppression
