beta blockers (HF) Flashcards
What do beta blockers inhibit?
chronic SNS activation in HF
What do they block?
beta adrenoceptors
What patients should use them?
- patients with LV systolic dysfunction (LVEF < 40%)
- asymptomatic with LV systolic dysfunction after MI
- stable NYHA 2-4
3 UK licensed agents
bisoprolol
carvedilol
nebivolol
How must patients be stable before treatment?
no physical evidence of fluid retention
no need for IV inotropic drug treatment
Where are beta blockers initiated?
in hospital
What should be assessed after every dose titration?
HR
BP
clinical ststus
side effects of beta blockers
- hypotension
- fluid retention -> worsening HF
- bradycardia (slow heart) / heart block
- fatigue
When should beta blockers be discontinued?
only in severe deterioration
What do beta blockers increase the density of?
beta 1 receptors
What do beta blockers inhibit?
cardiotoxic effects of catecholamines
What other good effects do they have?
- decrease neurohormonal activation
- decrease HR
