ARBs (HF) Flashcards
What do ARBs block?
AT 1 receptors
Why are ACEIs blunt instrumants?
there is angiotensin II escape during ACE inhibition
What effects does angiotensin II have?
bad effects at AT 1 -> aldosterone release, vasoconstriction, proliferative actions
good effects at AT 2 -> vasodilator, antiproliferative actions
What does decreasing vasoconstriction cause?
increases peripheral vasodilation
decrease SVR
What does decreasing aldosterone release cause?
decreases plasma volume
What does decreasing cardiac proliferative effects cause?
decreases cardiac remodelling
UK licenced agents
candesartan
losartan
valsartan
Indicated for what patients?
NYHA 2-4 who are intolerant of ACEIs
symptomatic NYHA 2-4 (on ACEI and beta blockers) who can’t tolerate aldosterone receptor antagonist
side effects
1st dose hypotension
worsening renal function
hyperkalaemia
