Referral
The referral can be:
- self referral
- third party referral
- professional referral
Self-referral
when you contact a care provider yourself
- self referral is usually done to primary services
Third-party referral
a person contacting a care provider on another persons behalf
Professional referral
when a care professional contacts another service to request support for a service user
Assessment
Local authorities have a duty to carry out a community care assessment
Eligibility Criteria
- a physical +/or mental impairment or illness
- an inability to achieve at least two of the following daily activities
- eat food
- wash themselves
- etc
Barriers to access
these barriers prevent services users from accessing a service, it may lead to service users not receiving the care they need and have a right to:
- language
- geographical
- financial
- scarce resources
- communication
- specific needs
Language
- if English isn’t service users first language
- no interpreter available
- hearing impaired and no signer available
Geographical location
- inconvenient location of service
- public transport
- cost of travelling
Financial
- potential loss of wages
- pay someone to look after young children
Scarce resources
- long waiting times
- lack of beds
- restricted opening times
Communication
- feel discriminated against
- feel prejudice against them
Specific needs
may not be able to ask for help, need an advocate, or miss appointments
Examples of specific needs
- people with learning difficulties
- people with ill mental health
- older people
- children
- refugees
- transgender people
