how does vitamin B12 work in the body?
- binds to intrinsic factor secreted from parietal cell
- complex is absorbed in terminal ileum
- transported via transcolbamin
where is vitamin B12 stored?
in the liver
name 3 causes of vitamin b12 deficiency?
- dietary
- intestinal malabsorbtion - crohn’s disease
- intrinsic factor deficiency
- autoimmune causes
what is the most common cause of vitamin b12 deficiency?
pernicious anaemia
what is pernicious anaemia?
- autoimmune chronic atrophic gastritis
- autoantibodies directed at parietal cells and intrinsic factor
3 –> lack of vitamin b12 absorbtion
which other conditions is pernicious anaemia associated with?
- autoimmune thyroid disease
2. increases the risk of gastric carcinoma
clinical signs of vitamin b12 deficiency?
1 .lemon colored
- ‘pallor’
- ‘jaundice’
( due to haemolysis)
- Weight loss
- Glossitis
- peripheral neuropathy
How does B12 deficiency affect the brain?
- neurological abnormalities
- peripheral neuropathy - Psychiatric disturbance
Treatment of B12 deficiency?
IM B12 injection every 3 months
