Progressive, irreversible condition involving the loss of articular cartilage, causing pain and deformity

What is rheumatoid arthritis?
Systemic autoimmune disorder with inflammatory synovitis that destroys cartilage
More common in females
Symmetrical distribution
What are the diagnostic criteria for rheumatoid arthritis?
Morning stiffness for 1 hour
Arthritis of 3+ joints for 6 weeks
Systemic arthritis for 6 weeks
Rheumatoid nodules
+RF factor
Radiographic changes
What is the RF test?
IgM antibodies against the Fe portion of IgG
HLA-DR4
What is the treatment for RA?
NSAIDs
Steroids
DMARDs
Surgery
Where are rheumatoid nodules located?
PIP and MCP
More proximally than OA
What is septic arthritis?
Infection in the joint
Metalloproteases cause permanent destruction in about 8-12 hours
Most common in kids from hematogenous spread
What is the hallmark diagnosis for septic arthritis?
Joint aspirate
> 50,000 WBC/mm3
What is the most common cause of septic arthritis in neonate to teens?
S. Aureus
What is the most common cause of septic arthritis in adolescents?
Gonorrheoeae
What is the most common cause of septic arthritis in adults?
S. Aureus
What are the most common joints and causes for ankylosing spondylitis?
SI joints, spine, Stiff joints
Eveitis, Carditis, Enthesitis
What are the most common causes for Reiter Disease?
Urethritis, Cervicitis, Dysentery
What is the treatment for seronegative spondyloarhtropathies?
NSAIDs, MTX
