Approach to Shoulder Complaint Flashcards
What is the only joint in the human body where tendons (rotator cuff) pass between bones (acromion and humerus)?
Shoulder
What is the most sensitive indicator of joint disease?
ROM
Special tests for subacromial impingement and rotator cuff tendon injury
- Painful Arc Test
- Neer Impingement Sign
- Hawkins Impingement Sign
- Empty Can Test
- Drop Can Test
Special Test for AC Joint/Ligament Injury
- Cross Arm Test
Painful Arc Test
- Positive: shoulder pain from 60-120
- Indicates subacromial impingement and/or rotator cuff injury
Glenohumeral Dislocation (Dislocated Shoulder)
- anterior dislocation most common
Acromioclavicular Joint Injuries
- occurs from direct trauma to the superior or lateral aspect of the shoulder (acromion) with the arm adducted, such as a direct blow or falling onto the shoulder
Rotator Cuff Injury
- supraspinatus is most commonly injured
- most common cause of shoulder pain in primary care
Major shoulder ROM comes from linkage and coupled motion from 3 joints:
- Glenohumeral Joint
- Acromioclavicular Joint
- Sternoclavicular Joint
Shoulder ABduction -> Scapula Elevation -> __ glide at AC joint & ___ glide at SC joint
- AC Joint: Superior (clavicle is superior to acromion)
- SC Joint: Inferior (clavicle is inferior to sternum)
Shoulder ADduction -> Scapula Depression -> ___ glide at AC joint & ___ glide at SC joint
- AC Joint: Inferior (clavicle is inferior to acromion)
- SC Joint: Superior (clavicle is superior to sternum)
Shoulder external rotation produces ___ rotation of the clavicle at the AC joint
External
Shoulder internal rotation produces ___ rotation of the clavicle at the AC joint
Internal
Scapular Protraction -> __ glide at the AC joint & ___ glide at the SC joint
- AC Joint: Anterior
- SC Joint: Posterior
Scapular Retraction -> __ glide at the AC joint & ___ glide at the SC joint
- AC Joint: Posterior
- SC Joint: Anterior
