Antidepressants Flashcards
Desipramine
TCA- 2ary amine (less sedation/antichol, more NE)
Imipramine
TCA -3ary amine (inc sedation, inc 5HT, inc anticholinergic)
For enuresis
Amitriptyline
TCA- 3ary amine (more sedation)
Nortriptyline
TCA - 2ary amine
Least cardiac effects
Timipramine
TCA- 3ary amine
Doxepine
TCA- 3ary amine
Clomipramine
TCA - 3ary amine
For OCD
Protriptyline
TCA- 2ary amine
Phenelzine
MAOI
Tranylcypromine
MAOI
Fluoxetine
SSRI - Prozac
LONG T1/2 (4wks to reach ss, 5wks to get rid off)
Inhibits CYP2D6- interactions w/ TCA, warfarin, phenytoin/carbamazepine, bb, opoids, tamoxifen, tramadol (seizures)
Fluvoxamine
SSRI
Paroxetine
SSRI + OCD and social anxiety
Inhibits CYP2D6 - DI w/ TCA, warfarin, phenytoin/carbamazepine, BB, opoids.
Some sedation, GI side effects. NOT safe in pregnancy
Sertaline
SSRI
Similar to Fluoxetine, less DI, shorter T1/2
Citalopram
SSRI
-Fewer side effects and DI
useful in hot flashes
Escitalopram
SSRI - fastest onset
Fewer side effects and DI
Useful in hot flashes
Venlafaxine
SNRI - for chronic pain
Inhibits reuptake of 5HT & NE
Inc BP
Desvenlafaxine
SNRI
Duloxetine
SNRI - depression associated pain
Inc liver enzymes, some CYP2D6 inhibition
Milnacipram
SNRI - fibromyalgia
Induces liver enzymes
Levomilnacipram
SNRI
Bupropion
Anti-depressant - inhibits DA reuptake
Smoking cessation (ext release), alcoholism, night eating, ADHD
FEWER SEXUAL SIDE EFFECTS, can give anxiety
CI: seizures or head trauma
Mirtazapine
Anti-depressant - blocks presynaptic a2, blocks 5HT2a & 5HT3
no cardiac toxicity
sedation (antihistamine), wt gain
Atomoxetine
Anti-depressant - blocks NE reuptake
ADHD med for <6yo
CI w/ vasopressors (epi, ne) and MAOIs
GI pd, hepatotoxicity
Maprotiline
Anti-depressant - blocks NE reuptake
cardiac effects, seizures
Amoxapine
Anti-depressant - blocks NE (&5HT) reuptake, blocks DA receptor
Some antipsychotic effect, sedation
Traxodone
Anti-depressant (not very good) - partial agonist at 5HT1a, blocks 5HT2a
For pain management and insomnia
sedation, PRIAPISM
St Johns Wort
Anti-depressant - effective for mild depression
Photosensitivity, don’t combine w/ other antidepressants, harder to get out of anesthesia
HTN crisis
When combine MAOIs w/ tyramine (wine/cheese) or sympathomimetics (cold meds) or Buspirone.
Tx w/ phentolamine (a-blocker)
Serotonin syndrome
Hyperpyrexia, convulsions, twitches, delirium
If combine MAOIs w/ TCAs, SSRIs, Meperedine, or Dextromethorphan
If combine SSRIs w/ MAOIs, St John’s wort, amphetamines.
Tx w/ benzos
SSRIs
DOC for depression, panic disorder, OCD
Also used in social anx, bulimia, PMDD, alcoholism, premature ejac
No memory impairement- can use in children/adolescent/elderly
Side effects: GI, photosensitivity, wt gain (chronic)
Selegeline
MAOI, selective for MAO-B (breaks down DA)
Vortioxetine
SSRIs + agonist at 5HT1a- anxiolytic
Viladozone
SSRI + agonist at 5HT1a - anxiolytic
