Anthracyclines Flashcards
MOA of anthracyclines
- Intercalation into DNA
- INHIBITION OF TOPOISOMERASE 2
- Creation of oxygen-free radicals that damage cells
Anthracycline drugs
Doxorubicin (Adriamycin)
Daunorubicin
Mitoxantrone
Safety concerns for doxorubicin
- CARDIOTOXICITY
- RED discoloration of body fluids
To prevent cardiotoxicity what is the TOTAL LIFETIME CUMULATIVE DOSE
450 - 550 mg/m2
When the doxorubicin cumulitive dose is ≥ 300 mg/m2 with planned continued treatment, what chemoprotectant should be used
DEXRAZOXANE
At what cumulative dose should the chemoprtoectant- Dexrazoxane be used if treatment will still be used
≥ 300 mg/m2
When should treatment of doxorubicin be stopped
When the cumulative dose reaches 450 - 550 mg/m2, tx must be stopped
(the lower end of the dose range is used if CV risk or previous mediastinal radiation)
Q. What should be monitored before and after tx with doxorubicin
Q. using what
- LVEF
- Echocardiogram or MUGA scan
Which anthracycline has a safety concerns specifically for BLUE discoloration of bodily fluids
Mitoxantrone
