Alkylating Agents Flashcards
Name alkylating agents
Cyclophosphamide
Ifosfamide
Busulfan
Carmustine
MOA of alkylating agents
Cross-linking DNA strands, which inhibits DNA and protein synthesis
Which drug(s) cause hemorrhagic cystitis
Cyclophosphamide and Ifosfamide
How is hemorrhagic cystitis caused
Caused by the toxic metabolite ACROLEIN which concentrates in the bladder
What monitoring parameters are used for hemorrhagic cystitis
HEMATURIA, lower urinary tract sx’s (urinary urgency, DYSURIA)
What is the prevention for hemorrhagic cystitis
- Adequate HYDRATION
- use of a chemoprotectant (MESNA)
When should Mesna be given
with ALL Ifosfamide doses and HIGH DOSES of Cyclophosphamide ( > 1 gram)
Q. Which alkylating agents cause pulmonary toxicity
Q. What is the Tx
- Busulfan and Carmustine
- Symptomatic care (ex: oxygen)
Which alkylating agents cause Neurotoxicity
Ifosfamide and Carmustine
Q. Which alkylating agents can cause seizures
Q. What is the prevention/ treatment
- Busulfan and Carmustine
- Anti-seizure medications (ex: levetiracetam)
What is the prevention/ treatment for Ifosfamide neurotoxicity (encephalopathy: confusion, incordination, imbalance)
Methylene Blue
