#Incidence to fatal - only 0.05%cases are severe in that very few are fatal
1) over dosage of either anaesthetic or vasoconstrictive agent
2) rapid absorption from absorptive areas or due to local vasodialtion
3) accidental injection to vessel
4) hypersensitivity reactions

Excitatory - cause convulsions
Depressive - respiratory paralysis
Rarely - heart effected or abnormally high concentration injected directly into nerve,loss function permanently
