What is alternate day therapy used for in steroid therapy?
Maintain therapeutic benefit previously achieved by daily steroid treatment
What drugs are used in alternate day therapy?
Intermediate acting steroids like prednisone
What are topical steroids used for?
Suppress inflammation and allergic manifestations of may skin disorders
What percentage of topical steroids are absorbed systemically?
1%
What is the efficacy of a topical steroid preparation a function of?
the potency and concentration of the steroid
Which are the aerosol corticosteroids?
- Mometasone
- Flunisolide
- Triamcinolone
- Beclomethasone
- Fluticasone
- Budesonide
What do the aerosol corticosteroids do?
They all inhibit synthesis of leukotrienes, prostaglandins, and thromboxanes which leads to reduction in bronchiolar smooth muscle contraction, vascular permeability and airway mucus secretion.
They also restore the responsiveness of bronchial smooth muscle to beta adrenergic agonists
What are the adverse effects of aerosol corticosteroids?
Less than oral corticosteroids.
- Cause changes in oropharyngeal microflora that may lead to infections such as oral candidiasis because you inhibit the immune system so you don’t fight these fungi.
What is the short term and long term adrenal stress response?
Short term: Release epinephrine and norepinephrine from the adrenal gland
Long term: Release of Glucocorticoids (conversion of protein and fats to glucose, increase blood sugar, suppression of immune system) and Mineralocorticoids (Aldosterone to retain sodium and water, increase blood volume and pressure)
