Anti-inflammatory mechanisms of GCs (4)
- enhance vasoconstriction –> decrease redness and swelling
- inhibit arachidonic acid metabolites–> redue fever and pain
- complex leukocyte effect–> decrease inflammation and defense
- Alter release of cytokines and inflammatory mediators
short duration, GC and MC effects, mild inflammation
hydrocortisone
intermediate duration, less MC effects
Prednisone
long duration little MC effect

MC drug
Fludrocortisone
MC antagonist, diuretic

Most commonly used GC drug

GC more potent due to lack of protein binding

Can be taken orally, parenteral, localized delivery important for anti-inflammatory effects
MC adverse effects
water retention, hypertention, edema
hypernatremia (high Na+), hypokalemia (low K+), alkalosis (low H+)
Most important contra-indications of GCs
heart disease, hypertension, diabetes, peptic ulcer, infections, osteoperosis, glaucoma, psychoses, all use in children
Critical therapeutic issues
may become dependent, must decrease dose slowly
