Complete FRD-072
Vehicle Accident with Collision (Property Damage Only)
Report immediately to DPSC (Sends BC/SAFO/PD)
Get name, address, insurance and policy # of other driver and witnesses
Provide Inc#, Email for Risk management
Complete Safety Incident Reporting System to include: Citizen Property Damage Report (Risk05) / FRD-317 / FRD-072 / Risk-03 if damaged equipment on unit
Contact Duty Apparatus and M5
Vehicle Accident with Collision (injuries)
Report immediately to DPSC (Sends BC/SAFO/PD)
*Call for additional resources for injuries
*Treat and transport any injuries and complete appropriate epcr reports
Get name, address, insurance and policy # of other driver and witnesses, Provide Inc#, Email for Risk management

Workplace Violence (Verbal or Physical Altercation)
Separate parties to different locations on site
*Request PD if necessary
Investigate what happened
Complete Workplace Violence Incident Form (FIN-03) and send to HR
It will then be sent to Professional Standards Office (PSO) and Risk Management
Harassment Complaint
Immediately Stop the behavior
Investigate behavior
Report it to direct supervisor and OHREP
Have complaint filed directly with EEOC
Romance with other personelle
Employees developing a romantic or sexual relationship while assigned to the same work location or section shall be required to immediately notify their immediate supervisor to
determine whether reassignment is required.
Employees with family members, household members, or extended relationships must not be assigned to the same office, unit, or station on a given shift and/or work location.
Request memos on relationship and send up chain for reassignment.
Drugs/Alcohol Impairment (Fitness For Duty)
Stop immediate action and relieve them of duty
Keep employee at work location, dont let them eat or drink
Notify BC and DFC, and complete FRD-025 Fitness for Duty
Notify Professional Standands Office
Employee is placed on admin leave after testing and will remain until results return
Fire Chief or designee makes determination after results if employee returns to duty, will need to be cleared by PSOHC (FRD-158) work status form
Physical / Psychological Impairment (Fitness For Duty)
Notify BC and DFC and consult findings / concerns
Relieve employee of duties and assignment
Fill out FRD-025 (Fitness for Duty) stating reasons for relieving employee

Check for Injuries
Notify DPSC
Notify Receiving facility
Notify EMS Capt / SAFO
Investigate Cause (Equipment failure or personnel failure)
Cot OOS
Document (FRD317 And Memo)
Missing or Damaged Knox Box Keys
Immediately notify Capt II of Operations Bureau / BC / DFC
Document in log book
Fill Out FRD-048A (Rapid Entry System Key Replacement)
Deliver form and broken keys in person
Fill Out FRD-048 (Rapid Entry System Key Issuance)
Loss Property
Complete Risk-03 within 24hrs
SAR if it could be used nefariously
Police report if stolen or vandelism
