*Treat and transport any injuries and complete appropriate epcr reports

Complete Workplace Violence Incident Form (FIN-03) and send to HR
It will then be sent to Professional Standards Office (PSO) and Risk Management
Harassment Complaint
Immediately Stop the behavior
Investigate behavior
Report it to direct supervisor and OHREP
Have complaint filed directly with EEOC
Romance with other personelle
Employees developing a romantic or sexual relationship while assigned to the same work location or section shall be required to immediately notify their immediate supervisor to
determine whether reassignment is required.
Employees with family members, household members, or extended relationships must not be assigned to the same office, unit, or station on a given shift and/or work location.
Request memos on relationship and send up chain for reassignment.
Drugs/Alcohol Impairment (Fitness For Duty)
Stop immediate action and relieve them of duty
Keep employee at work location, dont let them eat or drink
Notify BC and DFC, and complete FRD-025 Fitness for Duty
Notify Professional Standands Office
Employee is placed on admin leave after testing and will remain until results return
Fire Chief or designee makes determination after results if employee returns to duty, will need to be cleared by PSOHC (FRD-158) work status form

Personnel Arrives Late (Time and Attendance)
Supervisor may deem tardiness excused due to circumstances
Should be notified at least 1hr in advanced of being tardy, give a reason for being tardy, and expected arrival time. Tracks for 365 days of previous incident
1st Time: LWOP for time to nearest 1/10 of an hour / and Oral Reprimand
2nd Time: LWOP for time to nearest 1/10 of an hour / and Written Reprimand
3rd Time: LWOP for time to nearest 1/10 of an hour / and Proposed suspension for 2 days
4th Time: LWOP for time to nearest 1/10 of an hour / and Proposed Dismissal
Personnel Is No Call / No Show (Time and Attendance)
Fail to report to duty or notify officer within 1 hour after start time
1st Time: Arriving within 1hr = LWOP 1/10 of an hour and
Written Reprimand

If more than 4hrs: you don’t have to report to work the same day
If you’re scheduled to serve the day after a work day: you must be released by 1700 and placed on CVL Leave
If required to call a jury check-in number during a scheduled work day: you should be placed on CVL until determination of jury status is made. If required: stay on CVL. If not required: report to work asap.
