Acute oncology Flashcards
Name the different mechanisms for cord compression
Common:-
Crush fracture
Soft tissue tumour extension
Less common:-
1-Direct extension from malig retroperitoneal or mediastinal nodes eg lymphoma
2-extra dural mass in absence of bony involvement
3- rarer intramedullary mets or primary tumour
The five most common cancers for cord compression
Breast Prostate Lung Myeloma Lymphoma
Which site is most common?
Cervical
Thoracic
Lumbar
Sacral
Thoracic
Most common biochemical cause of AKI in tumour lysis?
High calcium
Used to be Uric acid but now we use prophylaxis
In TLS when should you give calcium supplements?
Only when symptomatic
Name the common chemotherapy drugs which are vesicants: 4 drugs/categories
Paclitaxel
Oxaliplatin
Mitomycin
Vinca alkaloids
When should a surgical opinion be sought for extravasation?
Vesicant drug
Conservative management fails to improve symptoms
Tissue damage occurs
Which chemo extravasation calls for warm compress
VPT
Vinca alkaloids (V-W)
Platins
Taxanes
Which chemo extravasation calls for cold compress
Anthracyclines
Mitomycin
AM cold in the morning
In hyponatremia what is the maximum in mmol/l the sodium should inc per day?
12
RECIST target lesions have to be..
Can measure
At least one dimension is >= 1cm
Partial response PR RECIST criteria
At least 30% decrease in sum of diameters of target lesions- taking as reference the baseline sum diameters
Progressive disease in RECIST
> = 20% increase in sum of diameters of target lesions
Reference is the smallest sum diameter recorded since treatment started
Must also have absolute increase of 5mm OR the appearance of one or more lesions
