What is acromegaly?
It is the clinical manifestation of excessive growth hormone
What produces growth hormone?
Anterior pituitary
What does growth hormone act on?
The liver to produce IGF-1
What does IGF-1 cause?
Excessive bone and soft tissue growth
What are the causes of acromegaly?
Pituitary adenoma
Ectopic GHRH or GH from pancreatic tumour
What is the presentation of acromegaly?
Headaches
Coarsening of facial features
Shoes no longer fit
Spade like hands
Why might the patient present with excessive sweating and oily skin?
Due to hypertrophy of the sweat gland
What signs would there be if the Acromegaly was caused by a pituitary tumour?
Bitemporal hemianopia
Hypopituitarism
Headaches
What are 2 important signs of Acromegaly?
Skin tags
Profuse sweating
What is the main test for Acromegaly?
Serum IGF-1
What is serum IGF-1 used for?
To test for Acromegaly
To monitor Acromegaly
What is used to confirm the diagnosis of Acromegaly?
Oral glucose tolerance test
Why would you do an MRI in Acromegaly?
For a pituitary adenoma
What is the management of acromegaly?
Trans-sphenoidal surgery
What would you give a patient not suitable for surgery?
Somatostatin analogue
