What is a hazard?
Work place condition that might result in injury, health impairment, illness, disease, or death to any worker who is exposed to the condition. Or might result in damage to or losss of property or equipment
What is a hazard severity?
Assessment of the worst potential consequence is likely to occur as a result of deficiencies
What is an injury?
Traumatic wound or other condition of the body caused by external force including stress or strain
What is a mishap?
Unplanned or unexpected event causing death, injury, occupational illness, including days away from work, job, transfer, or restriction and material loss or damage
What is a mishap probability?
Likelihood that a hazard will result in a mishap or loss
What is a near mishap?
An act or event which injury or damage was avoided merely by chance

Web-based safety mishap data collection and reporting system

What is the purpose of Mishap Investigation and Reporting?
- Indentify hazards with potential to cause frequent and severe mishaps
- Indentify hazards that could cause future mishaps
- Identify unsafe acts and conditions
- Indentify mishap causal factors and develop appropriate corrective actions
What conditions require Investigation and Reporting?
- Class A,B, and C gov’t property damage mishaps
2. Class A,B and C on duty DOD civilian mishaps and military on/off-duty mishaps
What specific conditions need to be reported?
- Any work related illness or injury that involves medical treatment beyond first aid
- All on-duty military training-related fatalities and any high or moderate risk training mishaps that result in:
a. Loss of one training day
b. Rolling back or disenrollment of the student from a course
Are needle sticks required to be reported?
Yes, any need-stick injury or cut from a sharp object that is contaminated with another person’s blood
Does work-related tuberculosis infection need to be reported?
Yes
What is Class A Mishap?
- Resulting total cost of damages to Gov’t and other property is $ 2 million
- DOD aircrafe is destroyed
- Injury or occupational illness results in a fatality or permanent total disability
What is a Class B Mishap?
- Resulting total cost of damages to Gov’t and other property is $500,000, but less than $2 million
- When 3 or more personnel are hospitalized
What is a class C Mishap?
- Total cost of property damages to Gov’t and other property is $50,000 or more, but less than $500,000
What is a Class D Mishap?
- Damages is $20,000 or more but less than $50,000
2. Recordable injury or illness not otherwise classified as Class A,B, or C Mishap
Who is responsible for submitting an Accident and Injury Report?
- Medical Dept will submit an A&I Report as directed by the CO, on all injuries treated by sickbay
- Copy goes to the Safety Officer
What is a SITREP?
- Safety Investigation Report
Who is the SITREP submitted to?
- Naval Safety Center

Why are Hazard Reports Submitted?
Submitted for near-mishaps and situations whre lessons learned should be shared
