8.1 - What would you do if you encountered a safety issue during commissioning?
A
- Stop work immediately and ensure the safety of yourself and other personnel, informing others to stop their operations.
- Assess the situation, evaluating the nature and extent of the safety issue and evacuate the area if necessary.
- Notify the relevant personnel i.e. deputy site manger, HSE, authorised engineer. Communicate the issue clearly, informing them of the issue, the location and what is affected.
- Document the incident and report it. Record details of the safety issue, time, location and circumstances surrounding.
- Await instruction from relevant notified person.

8.2 - If a client is unhappy with a commissioning outcome, how would you handle the situation?
A
- Arrange a meeting to discuss client concerns and allow them to express their feelings.
- Validate their concerns and acknowledge their feelings, taking their feedback seriously.
- Clarify their concerns and understand the specifics of their dissatisfaction. This can help pinpoint the root cause of the problem. Apologise and take responsibility if necessary.
- Propose solutions and try to come to a resolution. Ensure the client is happy with the solution before implementing.
- Maintain communication with regular updates to build trust and ensure the client feels valued.
- Learn from the experience and reflect, this can help prevent similar issues in the future.
