What is pulmonary fibrosis
scarring of lung parenchyma
What happens to the spirometry of someone with pulmonary fibrosis?
normal rate of flow
limited RV and TLC
What is sarcoidosis?
abnormal collections of inflammatory cells, usually starting in the lungs
aetiology of pulmonary fibrosis
widespread - drugs
upper - TB, EAA, sarcoidosis
lower - occupational, connective tissue diseases
How might the causes of pulmonary fibrosis be identified?
autoantibody screening test
What are the two main symptoms of fibrosis?
breathlessness
fine end inspiratory creps at lung bases
What would be seen on a fibrosis x-ray?
honeycombing
How would you treat UIP?
UIP - usual interstitial pneumonia
Pirfenidone or nintendanib
immunosuppression
transplantation
Which has a better prognosis, NSIP or UIP?
NSIP - non-specific interstitial pneumonia
UIP - usual interstitial pneumonia
NSIP has a better prognosis
What is EAA, what is is caused by, and what should it not be confused with?
Extrinsic Allergic Alveolitis
inhalation of organic particles
confused with asthma
What is granulomatosis, and what might cause it?
just inflammation really, encompasses alot
bugs (TB, fungi)
inflammatory (sarcoidosis, EAA)
Vascularic
What is Lofgrenis syndrome?
bilateral enlarged lymph nodes
