Therapeutic dosage of anticoagulation
What medication is used to prevent PE?
Prophylactic dose of low molecular weight heparin
What are three ways to prevent PE besides medication?
- Early mobility
- Compression stockings
- Intermittent compression devices
What are mobility goals for PE prevention?
Get the patient up and ambulating as soon as possible, bed rest is a major risk
What is an IVC Filter?
Inferior vena cava filter - prevents clots from traveling
What are the three medications for PE?
- Low molecular weight heparin
- Heparin
- Coumadin
What is indicated if INR is between 4.0 - 5.0 with Heparin or Coumadin?
Resistive exercises should be held and participation in light exercise is okay
What is indicated if INR is > 4.0 and gait is unsteady with Heparin or Coumadin?
Ambulation should be restricted due to risk of bleeding if a fall occurs
What happens as INR increased above 5.0 with Heparin or Coumadin?
Likelihood of bleeding increases quickly
What is indicated if INR is > 5.0 with Heparin or Coumadin?
Talk with MD is any mobility is warranted due to increased risk of hemorrhage
What is indicated if INR is > 6.0 and gait is unsteady with Heparin or Coumadin?
Bedrest until INR is corrected
When INR indicated bedrest, how long before levels normally are corrected?
Within 2 days
What is the HAS-BLED bleeding score?
Scale to assess the risk of hemorrhage for VTE
What are four interventions for pneumonia?
- Antibiotics
- Supplemental O2
- Mobility and endurance
- Breath timing and control
