A
sudden onset unilateral flank pain
- radiates anteriorly and inferior
- writhe and cannot find a comfortable position

Phx findings
A
- typically afebrile with normal vitals
- unilateral CVA tenderness
- no abdo tenderness

A
uncommon in elderly
- AAA
- aortic dissection
- renal art. thrombosis
- append
- pyelo

what is best lab and findings
A
urine dip - micro hema - no WBCs - no nitries HCG for all women

when to image
A
presumed first presentation
- low threshold for elderly

5 imaging modalityes
A
- CT - preferred
- good and can pick up some mimickers - US
- only modest sensitivity
- good for hydronephrosis - MR
- poor sensitivity for stones, but good for obst - IVP
- KUB xray
- only really good for follow up

Tx steps for colic
A
- IV morphine
- naproxen
- tamulosin
- IV saline

4 absolute indications for uro consult
A
- intractable pain or vomiting
- obstructed stone with infection
- single kidney
- transplanted with obstruction

prognosis for stones
A
80% will pass
over 7mm unlikely to pass
