Insufficient and unprecise information.

Money is lost due to the production stopping and the repair costs, also this could represent a risk for people.
What is the case of the Heilbronn Phantom?
DNA had been found in cotton swaps in several countries from 1993 to 2009, and supposedly this came from a killer related to several crimes ranging from burglaries to homicides. The police from three countries looked for the killer and in the end it resulted that the DNA came from a worker in the factory that manufactured the cotton swaps.
Similar to Heilbronn case in teacher’s former company.
The teacher had a young engineer that demonstrated in CATIA that a collision could not happened by doing a simulation. The company spent 100 thousand Euros in investigating the reasons of the collision in France.
Root Cause: There was only a problem with the painting that happened during the mounting of the part, no collision.
Characteristics of root cause:
Clearly a major cause of the problem, no better alternative to the cause.
It can be resolved and resolution will not create new problems.
What is the 5why technique?
Used in the 6 Sigma Analysis. Developed by Toyota’s founder.
A series of 5 whys? Why did my car stopped? Why didn’t it has gas? …
