Delegation
- Transfer of responsibilities for the performance of a task from one individual to another while retaining the accountability of the outcome
5W 1H of Delegation
Who
- You can delegate to other RNs, technicians, unlicensed assistive personnel (UAP), Licensed Practical/Vocational Nurses (LPN/LVN).
- RN’s are still accountable for the supervision needed
What
- PO medications can be delegated to LPN/LVN
- Ambulation can be delegated to UAP

- Right task, right circumstance, right person, right direction/communication, under the right supervision and evaluation.
What can LPN do
- Gather data and make observations such as breath sounds and pulse ox.
- Obtain wound cultures, ostomy care, catheter insertion, wound care, blood glucose, EKG’s.
- They can administer medication
- LPN’s cannot complete in depth assessments, initiate nursing care plans, or evaluate patients abilities.
CNA’s
- Provides patients with basic care and assist in everyday activities (such as bathing).
Nursing Assistants
- Measure Vitals
- Document Intake/Output
- Cannot do activities that include nursing judgment, such as assessing or evaluating.

- Other RN’s

During Delegation
- Consider individual education, training and experience
UAP Delegation
- ADL’s
- Bathing, Grooming, Dressing, Toileting
- Positioning
- Ambulating
- Bed Making
- Feeding (without swallowing restrictions)
- Vital Signs for stable clients
- Specimen collection
- Intake and Output
LPN Delegation
- Reinforce client teaching from standardized care plan
- Monitor client findings as input for RN’s ongoing assessment
- Tracheostomy care and suctioning
- Uncomplicated wound care
- Check NG tube patency
- Insert urinary catheter
- Administer medications (IV may be excluded)
- Administer enteral feedings
