Kotters 8 Steps for Change
1 - Create a sense of urgency
- Sparks motivation to help the team get moving
- Discuss opportunities that successful change can have
- Give people the chance to ask questions and get involved
2 - Powerful coalition
- Get a team together
- Mix of leaders and experts and stakeholders
3 - Create a vision of change
- Explain the values that are central to the process
- Inspire the future
4 - Communicating Vision
- Communicate your vision frequently and powerfully. Talk about it any chance you get
5 - Remove Obstacles
- Processes/structures that are holding back change and focus your energy there
- Recognize and award people for making change happen
6 - Creating short term wins, building on the change, and anchoring the change
Kurt Lewin’s 3-Step Change Model
Step 1 - Unfreeze your Organization
- CONVINCE those affected by the change that the change is really necessary
Step 2 - Change your Organization
- GET those who have accepted the need for change to implement it with you
Step 3 - Refreeze your Organization
- Take actions to reinforce and support the change so it becomes a permanent way of doing business
Diffusion of Innovation
- Overtime ideas gain momentum and diffuse throughout an organization (spread)
- As the idea diffuses, people may choose to either accept or reject the idea for use in practice.
5 Stages of Diffusion of Innovation
- Awareness (people become aware)
- Persuasion (people develop an attitude for the innovation either favorable/unfavorable)
- Decision (Person weighs the pros and cons of the innovation and decide to adopt/reject it)
- Implementation (Person puts the innovation to use, at this stage they are still determining how useful it is)
- Continuation (A person checks the results of the innovation decision and come to a final decision if they want to keep it)
Internal Barrier
- Restraining factors that are found within an organization or unit where change is occurring
- Older nurses with more experience do not want to adopt new protocols or a change in practice.
Early Adopter (Chamption)
- Promotes change early on and helps motivate other nurses for change
Planned Change
- Deliberate, Organized and has a goal for improvement
- Plan for sepsis protocol change includes education, focused sessions to discuss with the staff, and sepsis warning signs in various places on the unit.
