19. Dissection of Gluteal and Posterior Thigh Region Flashcards
What vessels can be found superficially medially in the thighs and knee?
Great saphenous vein and saphenous nerve
What are the cutaneous nerves of the thigh?
Lateral femoral cutaneous
Anterior cutaneous branches of femoral
Cutaneous branches of obturator
Which vein drains into the femoral vein?
Great saphenous vein
Which nerve emerges from the fascia lata distal to the knee to accompany the great saphenous vein?
Saphenous nerve
What makes up the following borders of the femoral canal?
- Medial
- Lateral
- Superior
- Floor
- Roof
Medial - medial border of adductor longus
Lateral - medial border of sartorius
Superior - inguinal ligament
Floor - pectineus and iliopsoas muscle
Roof - cribriform fascia
What is found in the femoral canal?
Fat and lymphatics
What is the clinical significance of the femoral canal?
Femoral canal clinical significance: femoral hernia (female more prevalent than male), access for vascular procedures/imaging
Which artery arises from the femoral artery in the femoral triangle?
The profunda femoris artery
What is unique about the sartorius muscle?
Starts on ASIS - then passes inferioromedially to insert on the tibia at pet anserinus - this means it crosses both the hip and the knee joint.
What is found at the apex of the femoral triable?
Adductor canal
What vessels are found in the adductor canal?
Femoral artery
Femoral vein
Saphenous nerve
What forms the quadriceps tendon?
The convergence of all 4 quadriceps muscles
What does the quadriceps tendon attach to?
The superior border of the patella
Which four muscles make up the quadriceps?
Rectus femoris
Vastus medialis
Vastus lateralis
Vastus intermedius
Which muscles do the anterior and posterior branches of the obturator nerve supply?
Obturator nerve divisions innervate:
a. Anterior – adductor brevis, adductor longus, pectineus, gracilis
b. Posterior – adductor magnus, obturator externus
What is the thickening of the fascia lata over the superior border of the gluteus Maximus called?
Gluteal aponeurosis
Which muscles lies deep under the gluteus Maximus?
Gluteus medius
What arises form the inferior border of the piriformis muscle?
The inferior gluteal artery and nerve
Which muscles lies deep to the gluteus medius?
Gluteus minimus
What arises from the superior border of the piriformis muscle?
Superior gluteal nerve and artery
What are the nerve roots of the sciatic nerve?
L4-S3
Which is the largest here in the body?
Sciatic nerve (has its own arterial supply!)
What does the sciatic nerve bifurcate into? Where?
Into tibial and common fibular neres
Sometimes in the gluts, but most commonly occurs proximal to the apex of the popliteal fossa.
When giving an intragluteal injection, how do you identify a safe area to administer it and which structure do you want to avoid damaging?
Safe area is identified by gluteal quadrants, aim for upper outer quadrant, avoid the sciatic nerve
Which nerve innervates the following?
- Gluteus medius
- Gluteus minimus
- Gluteus maximus
Medius = Superior gluteal nerve
Minimus = Superior gluteal nerve
Maximus = Inferior gluteal nerve
What are the nerve roots for the superior gluteal nerve?
L4-S1
What are the nerve roots for the inferior gluteal nerve?
L5-S2
What is the function of gluteus medius and minimus?
Abducts femur at hip
Medially rotates thigh
What is the function of gluteus Maximus?
Extends femur at hip
Laterally rotates and abducts femur at hip joint
Which nerve innervates the tensor fascia lata?
Superior gluteal nerve
