Ans. D Bat Droppings - The patient has Crytpococus neoformans ( and encapsulated yeast which narrow base budding yeast) it can be stained with India ink or silver stain it infects patients with CD4 bellow 100 and symptoms include: pulmonary symptoms as wells as malaise, headaches and photo phobia.
A.Cats- Taxoplasmosis and intracellular parasite. Usually has CNS symptoms also could be Bartonella which is a gram negative rod and would be noted for Angiomatosis
B. Injection of contaminated food- cyrtosporidosis which is an intracellular parasite
C. Sand Fly Bite- Leishmaniasis which is an intracellular parasite and note for “black fever” darkening of the skin.
E. Plaques in cracked teeth- would be MAC, patient is above threshold which is 50 CD4T. Symptoms mimic TB . MAC is an acid fast bacillus
