Endometriosis definition
Presence of endometrial tissue outside the uterine cavity
Hormone influencing endometriosis
Estrogen-dependent
% of female population with endometriosis
10% (almost exclusively reproductive ages)
MC s/sx of endometriosis
Dysmenorrhea; Dyspareunia; Pelvic pain, infertility. NOT COMMONLY associated with LEIOMYOMAS
What determines symptoms?
Depend on localization of endometriosis, depth of infiltration and if condition is complicated by adhesions
Typical endometrosis locations
Ovarian; Retrocervical; Deep bowel; Bladder; Abdominal wall
Gold standard for endometriosis dx
Laparoscopy
Other helpful imaging studies for endometriosis work up
MRI- helps determine depth of infiltrating endometriosis, esp when laparoscopy limited d/t adhesions
Endometriosis tx
Based on extent of dz, symptoms & parity.
Medications - NSAIDs; OCPs; Progestin only meds; GnRH agonists
Surgery - Removal of endometriosis implants; recurrence rate of 40-8-%
Hysterectomy/Oophorectomy - Last resort
