what problems does a left displaced abomasum cause?
decreased motility - hypocalcemia, increased concentrate feeding/decreased fiber
increased gas production
abomasum rises dorsally
what clinical signs may be seen in a cow with a left displaced abomasum?
scant but loose feces
change in abdominal shape - left sided ping at the 12th intercostal space & sunken left paralumbar fossa
what are some metabolic disturbances seen in a cow with a left displaced abomasum?
dehydration, metabolic alkalosis, hyponatremia, hypochloremia, hypokalemia = causes disruption in milk production
what are the goals in treating a left displaced abomasum?
return the abomasum to a normal position
prevent recurrence with pexy
minimize patient illness in order to preserve health/milking function
what are some treatment options for a left displaced abomasum?
roll the animal
left-sided abomasopexy
right paramedian abomasopexy
what are some indications for a paramedian exploratory laparotomy?
great visualization of the abomasum - correction of LDA, correct right volvulus of abomasum, & identify & correct abomasal adhesions or ulcers
palpations of rumen, reticulum, omasum, abomasum, pylorus, duodenum, kidneys, liver, & gall bladder
what are some contraindications of a right paramedian abomasopexy?
respiratory disease, late gestation/fairly early post-partum, & suspicion of disease in the caudal abdomen
what is the holding layer in a right paramedian abomasopexy?
external rectus sheath
