Group 3 power chair
Standard w/c, >35 lbs. Unable to ambulate, and impairment in mobility related to ADL.

Pediatric power w/c with single or multiple power options
Group 5 power chair
Lightweight w/c, 34-35 lbs. Cannot propel standard w/c in home, and client can and does propel lightweight manual w/c.
K0003
Ultra-lightweight chair, <30 lbs. Pt has sufficient UE strength and endurance to propel and maneuver w/c. Extensive use of w/c and activities are documented. Adjusts vertically and horizontally. Client unable to move in less expensive chairs.
K0005
High strength lightweight chair, <35 lbs. Activities that pt engages in cannot be performed in a standard or lightweight w/c and/or pt requires a seat width, depth or height that cannot be otherwise accommodated. Can adjust vertically only.

can add non-joystick controller and/or power options
Group 2 power chair
Standard hemi-height manual w/c, >35 lbs. Standard hemi (low seat) w/c.

power chair with added capabilities making it easier to go outside; not for use in home
Group 4 power chair
non-central nervous system diagnosis, endurance diagnosis, in home use
