what stage of dementia? deficits in one of these areas: Lost when traveling to unfamiliar locations, poor work performance, word/bane finding deficits, retain relatively little material when reading, decreased ability to remember names upon introduction, lost or misplaced an object of value, concentration deficit may be evident on clinical testing.
3: mild cognitive decline (early confusional)
what stage of dementia? Subjective c/o memory deficit No objective evidence of memory deficit on clinical interview; No objective deficits in employment or social situations; Appropriate concern w/respect to symptomatology
2: very mild (forgetfulness)
what stage of dementia? no decline

Deficits in decreased knowledge of current/recurrent events, deficit in memory of one’s personal hx, concentration deficit elicited on serial subtractions, decreased ability to travel to familiar locations. Inability to perform complex tasks. Denial is dominant defense mechanism.
4: moderate cognitive decline (late confusion)
Meaningful verbal abilities are lost. Frequently, there is no speech at all. Incontinence. Loss of basic psychomotor skills. Apraxia.
7: late dementia
Pt can no longer survive w/o some assistance. Unable to recall a major relevant aspect of their current lives during an interview such as address, phone number, names of close family members, name of high school or college. Frequent disorientation to time. Difficulty counting back from 40s by 4 or 20s by 2. Retain knowledge of many major facts regarding themselves and others. Require no assistance w/toileting and eating, but may have difficulty choosing proper clothing to wear.
5: early dementia
May occasionally forget name of significant other, generally unaware of all recent events and experiences in lives, retains some knowledge of their past lives. Difficulty counting from 10, backwards and forwards. Assistance w/ADLs. Almost always recall their own name. Frequently continue to be able to distinguish familiar from unfamiliar. Personality, emotional and behavioral changes occur Delusional/paranoid behavior, perseveration or obsessive behaviors, anxiety or agitation, loss of sustained purposeful activity
6: middle dementia
