EEG! What if he has seizures!
Rebound headache
Using triptans, ergots, and fioricet (combo of caffeine, butalbital and acetainophen) more han a few times per week => rebound.
Myopathy emg shows?
No insertional fibrillations, low amplitude small motor units, early recruitment of motor units but normal interferece pattern
Neuropathic EMG shows?
Fibrillation potentials on needle insertion, large motor units, reduced interference pattern when pt asked to contract more strongly
Best imaging for CNS mets?
Brain MRI! for number of mets and possible meningeal disease
Pseudotumor cerebri
ICH, treated by repetitive high volume LP and maybe shunting! Papiledema = enlarged blind spot w/ periphreal field constriction
Donezepil
= Aricept, cholinesterase inhibitor, modest benefits for pts w/ mild/moderate Alzheimer’s!
Toxoplasmosis
Pyrimethamine + sulfidizaine! Hperfelex, UMN signs, ring enhancing lesion!
