Osteoarthritis
Chronic, degenerative synovial joint condition characterized by loss of articular cartilage
abnormal load mechanical factors which affect loading and wear
transmission of inflammatory mediators from the synovial fluids into the cartilage causing inflammatory reactions and subsequent damage
chondrocytes attempt repair but there is excessive enzymatic cartilage degradation that leads to progressive chondrocyte death and cartilage destruction = subchondral bone becomes involved in the process
bone undergoes infarction in places forming bone cysts
attempts at repair = subchondral sclerosis and marginal osteophytes
synovium and periarticular ST become inflamed = joint looses mobility and function
Primary OA
no discernible or identifable aetiology
Secondary OA
where etiology can be identified
Ra patho
progressive, autoimmune disease.
occurs when there is a interaction between genetic and environmental risk factors of RA
antibodies flow into the synovial joint via the blood circulation, stimulate leucocyte proliferation.
Infliltration of the synovium with T-lympocytes and macropphages then release
Gout
inflammatory metabolic condition that occurs when a patient has a purine rich diet which leads to higher hyperuricemia
this results in decreased renal urate excretion and hence increased urate production which then results in monosodium urate crystals forming which then leads to the painful onset of a gout attack
