Transplant Process
Patient placed on waiting list after being deemed a candidate
Waiting list maintained by United Network of Organ Sharing (UNOS)
When an organ becomes available, is matched to patient based on various characteristics (age, blood type, etc.)
Organ is preserved and transported to the recipient
Indications for heart transplant
end-stage heart disease, NYHA Class III-IV heart failure, congenital heart disease, quality of life
Most patients who are candidate for transplants are
on maximal medical management protocols, cannot attend to functional tasks, and have limited life expectancy
Most common conditions leading to heart transplant
HF, CAD, cardiomyopathy
Relative contraindications to heart transplant
Age >72
Drug, tobacco, ETOH use within 6 months
Mental illness/psychosocial instability
Some neurologic disorders
Morbid obesity (BMI >35)
Cachexia
PVD
Uncontrolled DM
Current/active infection
Absolute contraindications to heart transplants
Severe obstructive pulmonary disease
Poor renal/hepatic function
Autoimmune/infiltrative disease with multisystem involvement
AIDS
Current/recent malignant cancer
Systemic illness with short life expectancy
Transplant evaluation team members
physician, nursing staff, rehab staff, dietitian/nutritionist, psychologist/counselor, social worker
Evaluation includes
Labs
Pulmonary function tests, CT, VQ scan
Exercise testing, EKG, echocardiogram
Reassessed while on transplant list
Evaluation goal
assessment of whether individual can be adherent to post-op and medication regimens, has adequate social support, can navigate stressors associated with surgery
