Fluoroquinolones: examples
Ciprofloxacin, Levofloxacin
Fluoroquinolones (-floxacin): activity
Broad spectrum w/ good coverage for gram (-) bacteria
E. coli, h. flu, m. catarrhalis, p. aeruginosa, s. pneumonia, mycoplasma
Fluoroquinolones (-floxacin): MOA
Bactericidal through interference w/ enzymes required for synthesis and repair of bacterial DNA and promotes breakage of DNA strands
Fluoroquinolones (-floxacin): ADR
HA, insomnia, dizziness, N/V/D, phototoxicity
Rare: hypersensitivity reactions, tendinitis, tendon rupture, c. diff
Fluoroquinolones (-floxacin): caution and contraindications
- BLACK BOX: tendon rupture and tendonitis
- Risk for QT prolongation
- Avoid in patients with myasthenia gravis
- Caution in renal and hepatic impairment
- Caution in elderly (dose reduce)
- Caution in pregnancy and lactation (avoid in patients <18 years)
Why can’t fluroquinolones be used to treat gonorrhea and TB anymore?
Increasing resistance because of overprescribing
No longer first line
Fluroquinolone: indications
- Complicated UTI, pyelonephritis, chronic bacterial prostatitis
- PNA/chronic bronchitis exacerbation
Fluoroquinolone: patient education
- Food delays absorption
- Take w/ full glass of water
- If tendon rupture occurs, d/c immediately and notify provider
