Inflammation has been shown to decrease response to antidepressants
HPA axis and cortisol levels have been shown to decrease the response to therapies.
But there is evidence that each also reduce reaction to treatment generally.

Prior Illness severity: Number of previously unsuccessful treatments predict worse outcomes.
