Warfarin Flashcards
What is an important substrate of CYP2C19?
Warfarin,…
Substrates of CYP2C9?
Ibuprofen, Plavix (Clopidogrel, anti platelete aggregation), Phenytoin
Location of CYP2C9
10q23.3
What is Warfarin?
Anti-clotting agent:
Warfarin activates VKOR -> VKOR converts Vitamin K in its reduced form and thereby reduces clotting
How does CYP2C9 metabolize Warfarin
CYP2C9 removes S-Warfarin
What is the effect of high/low amounts of CYP2C9 activity?
LOW activity: less removal of Warfarin, excess warfarin -> bleeding
HIGH activity: too much removal of Warfarin, increased clotting -> risk of stroke
Why is it important to monitor the level of Warfarin?
Because it has 40 hr of half-life
Optimal dose range for Warfarin?
2 mg/day to 10 mg/day
The first step of finding individual dose?
Measuring INR
