vonWillebrand's Disease Flashcards
1
Q
What does vWF do?
A
- Brings platelets into contact with exposed subendothelium
- Make the platelets bind to eachother
- Bind to factor VIII, protecting it from destruction in the circulation
2
Q
How are most cases inherited?
A
Autosomal dominant
3
Q
How does it present?
A
- Epistaxis
- Menorrhagia
- Bruising
- Bleeding gums
Like a platelet disorder
4
Q
What are the types of vWD
A
Type 1 - Partial reduction (most common)
Type 2 - Abnormal form of vWF
Type 3 - Total lack of vWF (autosomal recessive)
5
Q
What investigations are required?
A
- Prolonged bleeding time
- Raised APTT
- Low factor VIIIc
- Low vWF antigen
- Normal INR and normal platelets
6
Q
What is the management?
A
- Tranexamic acid for mild bleeds
- Desmopressin raises levels of vWF
- Factor VIII concentrate
