Visual Field Defects Flashcards
causes of visual field defects
vascular disease
SOL
demyelination (MS)
trauma/surgery
four areas that can be affected to cause a visual field defect?
- optic nerve
- optic chiasm
- optic tracts/radiations
- occipital cortex
causes of optic nerve damage?
CVS RF GCA altitude defect MS tumours e.g. meningioma, glioma and haemangioma
presentation of optic nerve damage
Progressive visual loss (unilateral)
pain on eye movement, most commonly caused by MS
painful loss of vision 1-2 weeks
Colour desaturation
Central scotoma (peripheral vision spared)
Gradual recovery over weeks-months. Optic atrophy can follow this.
what can affect the pituitary chiasm?
pituitary tumours
craniopharyngioma
meningioma
presentation of pitutiary chiasm defects?
bitemporal hemianopia
what affects the optic tracts?
tumours
demyelination
vascular abnormalities
presentation of optic tract damage
Homonomous defect
Macula not spared
Quadrantanopia
Incongruous
what can affect the occipital cortex?
vascular disease (CVA) demyelination
presentation of damage to the occipital cortex
Homonous defect
Macular sparing- large area/ dual blood supply
Congruous
where is pathology is a contralateral homonymous superior quadrantinopia is present?
temporal lobe
where is pathology in a contralateral homonymous inferior quadrantinopia?
parietal lobe
where is pathology in a contralateral homonymous hemoanopia (macular sparing)
visual cortex
where is pathology if a contralateral homonymous hemoanopia is present?
optic tract
