what is episcleritis?
inflammation of the thin vascular layer between the conjunctiva and sclera
what conditions is episcleritis associated with?
RA
sarcoidosis
IBD
presentation of episcleritis
unilateral redness
foreign body sensation
dilated vessels
management of episcleritis
topical lubricants
NSAIDs if severe
usually self-limiting and recover in 1-4 weeks
what is scleritis
inflammation of the sclera
what is more serious- scleritis or episcleritis?
scleritis
conditions associated with scleritis
HLA-B27 RA IBD sarcoidosis GPA
what can scleritis progress to?
necrotising scleritis that causes perforation
presentation of scleritis
pain on movement (can wake patient due to severity) tender on palpation photophobia abnormal reaction to light reduced visual acuity
management of scleritis
NSAIDs
steroids
immunosuppression
what is anterior uveitis?
inflammation of the iris and ciliary body
causes of anterior uveitis
AI HLA-B27 infection trauma ischaemia malignancy
causes of chronic anterior uveitis
sarcoidosis
syphilis
TB
herpes
presentation of anterior uveitis
unilateral sudden onset pain that can radiate to brow and exacerbated by focusing on near object red eye hyperaemia at limbus can develop hypopyon photophobia (BUZZ) small pupil and ptosis due to swelling and adhesions blurred vision
management of anterior uveitis
mydriatics (cycloplegics e.g. cyclopentolate hydrochloride) steroids immunosuppressants laser therapy, cryotherapy vitrectomy
