Ventricular Tachyarrhythmias Flashcards
Identify three pathological mechanisms of ventricular tachyarrhythmia
- Ventricular ectopic beats
- Ventricular tachycardia
- Ventricular fibrillation
What is torsade des pointes?
- Type of ventricular tachycardia
Identify two causes of torsades des pointes
- Congenital long QT syndrome
- Class 3 antidysrhythmics (amiodarone and sotalol)
What is Brugada syndrome?
- Type of ventricular tachycardia
- Caused by mutation in SCN5A gene
- Common in young males in south east Asia
What is seen on ECG in ventricular ectopic beats?
- Single wide QRS complex with no p wave
- Otherwise normal
What is seen on ECG in ventricular tachycardia?
- Wide QRS complexes
- Absent P waves
What is seen on ECG in Brugada syndrome?
- Coved ST elevation in V1-V3
What is seen on ECG in torsades des pointes?
- Broad complex tachycardia
- Alternating directions of QRS complexes
What is the treatment of ventricular ectopics?
- Not normally required
- Beta blockers if significant arrhythmia
What is the first line treatment for haemodynamically stable patients with VT?
- Intravenous amiodarone
What is the second line treatment for haemodynamically stable patients with VT?
- Intravenous lidocaine
What is the first line treatment for haemodynamically unstable VT?
- Emergency cardioversion
What is the first line treatment for haemodynamically stable VT?
- Intravenous amiodarone
What is the first line treatment for torsades des pointes?
- Magnesium sulfate
Outline the treatment options for acquired long QT syndrome
- Correct electrolyte disturbances
- Stop causative drug
- Magnesium sulfate
- Intravenous isoprenaline
