A-fib d/t dilation of the atrium Right side HF symptoms: Hepatomeglia (ascities) GI discomfort d/t increased fluids Peripheral edema JVD
S/S of Pulmonary Stenosis/Insufficiency
Same as Tricuspid Stenosis/Insufficiency
S/s of Mitral Valve Prolapse
Mitral click (systolic click)
Supraventricular dysrhythmias
Signs of HF
Tx of Mitral Valve Prolapse
Prophylactic Abx before invasive procedures. (Goes for all valvular disorders)
Tx of Supraventricular dysrhythmias (SVT- vagal, Ca channel blockers, adenosine)
S/S of Mitral Stenosis
Progressive fatigue Dyspnea on exertion Hemoptysis (coughing up blood) Repeat resp. infections A-fib (d/t hypertrophy, and dilation of the L atrium)
Tx of Mitral Stenosis
Positive inotrope (digoxin) Diuretics Afterload reducer in NOT used. Anticoagulation for A-fib Surgery
S/S of mitral insufficiency
Palpitations of the heart
SOB with exertion
Cough d/t pulmonary congestion
A-fib d/t dilation of L atria
Tx. of Mitral insufficiency
Diuretics
Afterload reducer to decrease SVR
Minimize use of interpose because they increase regurgitation.
Valve replacement
S/S of Aortic Stenosis
Exertional dyspnea d/t L ventricular dysfunction and pulmonary congestion.
Dizziness, fainting d/t decreased cerebral perfusion
Angina
Systolic Murmer
Tx. of aortic stenosis
Valve replacement
What is a unique post OP complication of valve replacement.
The rapid correction of hemodynamic pressures because the heart muscle has hypertrophied and dilated over time d/t valve disorder, and cannot rapidly reverse to normal.
Complications of valvular surgery
Bleeding Hemodynamic instability Emboli Infections HF HTN Dysrhythmias Mechanical Obstruction
Marfan’s syndrome can cause which valvular disorders
Aortic regurgitation and Mitral prolaps
