Mitral vale regurgitation (most common) bc of shortening of the leaflets.

Fever
Murmurs
Splenomegaly
Splinter hemorrhage
Osler’s nodes (nodules in the fingers or toes)
January lesions (hemorrhagic lesions on palms and soles)

IV Abx (penicillin) for 4-6 weeks 
Prevent emboli that can lead to stroke
