UTI, haemoturia Flashcards
UTI definition
symptomatic + positive culture/dipstick
Urethral syndrome
symptomatic but no bacteriuria
Classification of UTI
- Uncomplicated - normal GU tract
- Complicated- abnormal GU tract, obstruction, decreased renal function etc..
- Recurrent - new organism
- Relapse - same organism
Pyelonephritis symptoms
Fever
Loin pain
Vomiting
Oliguria
Cystitis symptoms
Suprapubic tenderness
Foul smelling urine
Prostatitis symptoms
Flu-like symptoms
Low backache
Dysuria
Tender swollen prostate on PR
Sterile pyuria definition
Increased white cells in urine but no bacteria
Sterile pyuria causes
TB
Treated UTI
Appendicitis
Calculi
Papillary necrosis
Polycystic Kidney
Chemical cystitis (e.g. cyclophosphamide)
UTI risk factors
Female
Sex
Pregnancy
Menopause
DM
Abnormal tract: stone, obstruction, catheter,malformation
UTI organisms
E. coli
Staphylococcus saprophyticus
Proteus (alkaline urine → struvite renal stones)
Klebsiella
Cystitis treatment
Treat for 3-6 days
Trimethoprim or Nitrofurantoin (not in Renal Failure)
Cefalexin (good in Renal Failure)
Pyelonephritis treatment
Cefotaxime for 10 days
No response: Augmentin + gentamicin
causes of false haematuria
- Beetroot
- Rifampicin
- Porphyria
- Vaginal bleeding
rare causes of haematuria
- Henoch-Schonlein purpura
- bleeding diathesis
renal causes of haematuria
- Infarct
- Trauma: inc. stones

urethra causes of haematuria
- Infection
- Stones
- Trauma
- Tumour
Haematuria at the beginning of micturition
urethral
Haematuria at the throughout micturition
Renal/systematic
Haematuria at the end of micturition
stone
schistosomiasis
causes of asymptomatic haematuria
- IgA Nephropathy/Berger’s disease
- Thin Basement Membrane Disease
- Alport’s syndrome
prognosis of IgA nephropathy/Berger’s disease
20% End stage renal disease after 20yrs
Genetics of thin basement membrane disease
Autosomal dominant
Presentantation of thin basement membrane disease
asymptomatic haematuria
Genetics of Alport’s syndrome
85% X-linked inheritance
Presentation of alports
Haematuria, proteinuria → progressive renal failure
- Sensorineural deafness
- Lens dislocation and cataracts
- Retinal “flecks”
- Females: haematuria only
