Renal tubular disease Flashcards
Renal tubular acidosis (RTA) def
metabolic acidosis caused by impaired acid secretion by kidneys
Type 1 RTA
Inability to excrete H ions in the distal tubule
Type 2 RTA
Defective HCO3 reabsorption in the proximal tubule
Type 3
type 1+2
Type 4 RTA
Low renin
Low aldosterone
Causes of type 1 RTA
SLE
Sjogrens
Amphotericin (antifungal)
Type 1 RTA sx
Rickets/osteomalacia (buffering of H with Ca)
UTIs
Calcium phosphate stones
Ix for type RTA
urine pH <5.5 despite metabolic acidosis
Rx of type 1 RTA
Calcium bicarb or citrate
Dx of type 2 RTA
IV sodium bicarb
high fraction of excreted HCO3 (>15%)
Rx of type 2 RTA
high dose bicarb
Type 4 RTA causes
Addisons
Diabetic nephropathy
Interstitial nephritis
Type 4 RTA rx
Rx cause
Rx hyperkalaemia
Fanconi syndrome pathophysiology
proximal tubular defect
Loss of amino acids, glucose, PO4, HCO3 in urine
Effect of Fanconi syndrome
dehydration
metabolic acidosis
osteomalacia/rickets
