define urticaria
aka hives
vascular rxn of skin w/ wheals surrounded by blanchable red halo or flare
cardinal sx is pruritis or itch
etiology of urticaria
type 1 hypersensitivity, swelling of upper dermis
papules/plaques last less than 12 hrs, may come up somewhere else
meaning of spongy, edema changes
deeper down in the skin
common causes of acute uriticaria
idopathic
post infection
food rxns
drugs or infusions
usually not detergents or contact allergens, except latex
etiology of chronic urticaria
usually idopathic
physical- pressure, heat, water, etc (maybe emotion)
chronic autoimmune- IgE receptor Abs
most common physical form of urticaria
dermatographism- localized edema or wheal w/i seconds
effector cell of urticaria
mast cell
immunologic vs non uriticaria
immunologic- antigen binds IgE on mast cell causing histamine release
non- direct mast cell degranulation w/o antigen
tx of urticaria
antihistamines- oral H1 are first line for acute and chronic
2nd generation less sedating
angiodema def
same pathogenesis of urticaria, but deep dermis w/ subq swelling
location for angioedema
face or an extremity- maybe tongue, pharynx, larynx, bowels
maybe painful but not pruritic
more serious condition assoc w/ urticaria and angioedema
anaphylaxis- need to ask about SOB, throat tightness, nausea, vom, abdominal pain
assess ABCs
tx for anaphylaxis
epi, IV fluids, oxygen
