acne vulgaris is a disorder of
pilosebaceous follicles
3 classifications of acne
comedonal- open and closed comedones
inflammatory- papules and pustules
nodulocystic- nodules and cysts
need to note severity of scarring
4 pathogenic factors
hormones- androgens
sebaceous gland activity
plugging of hair follicles from abnormal keratinization
P acnes bacteria in hair follicle living in lipids and causing inflammation
tx for acne, mild to sever
mild comedonal- topical retinoid
mild pap/putular- topical clinda or eryhtromycin
mod pap/pustular or mild nodular- oral antibiotics
severe scarring or nodular- isotretinoin
topical retinoids
vit A derivatives
can cause dryness, rednes, photosensitivity
3 types of topical antibiotics
erythromycin, clindamycin
benzoyl peroxide
takes 4-6 weeks to work
oral antibiotics for inflammatory acne
mino and doxycycline
side effects of oral antibiotics
mino- vertigo, vestibular toxiic
doxy- highly photosenstizing
GI upset
dont affect OCP
indication for oral isotretinoin
severe, nodulocystic, recalcitrant acne- low threshold w/ scarring
side effects of isotretnoin
chapped lips, dry skin, cheilitis, elevated LFTs, hyperTGs, psuedotumor cerebri
highly teratogenic
