seborrheic dermatitis is a response to
inflammatory rxn to malassezia yeast, thrives on seborrheic oil producing skin
pres of seb dermatitis
erythematous scaling patches on scalp, hairline, eyelids, face, central chest, behind ear, etc- basically dandruff
can be hypopigmented w/ darker skin
tx for seborrheic dermatitis
low potency topical steroids or ketoconazole cream for face
antidandruff shampoo for scalp and chest- can have ketoconazole, works best w/ longer contact time
pres of infantile seb dermatitis
“cradle cap”
diffuse yellow greasy scale on scalp, can affect neck, folds and diaper area w/ erythema
sites for rosacea
mid face and nose cheeks
sometimes brow chin, eyelids
pres of rosacea
erythema and telangiectasias
can have papules and pustuules, absence of comedones
can have rhynophyma- gland hyperplasia of nose
ocular rosacea- keratitis, blepharitis, conjunctivitis (can be harder to treat)
triggers for rosacea
EtOH, sunlight, hot foods, beverages
phymatous rosacea
papules, pustules on nose forehead cheeks and chin
thickened and sebaceous skin
tx for rosacea
depends on pt preference
topical metronidazole
oral antibiotics for pustular/papular lesions
isotretinoin for severe cases, sometimes surgery for rhinophyma
sunscreen
key sx to ask about for rosacea
ocular sx
perioral dermatitis pres and tx
perioral erythema and papules spare area near vermilion border
preceded by topical steroid use, tx w/ tetracyclines or macrolide for 1 month
key elements of lupus rash
often scaly/scarring
spares nasal creases
photodistributed
need to distinguish from rosacea
allergic contact dermatitis is an ex of
delayed type hypersensitivity
prototype is poison ivy
eyelid dermatitis causes/tx
atopic or contact
often from transfer from the hands- cosmetics, metals (nickel, gold), topical meds
milder steroid cream like desonide
test for delayed type hypersensitivity
patch test- common allergens applied on square patches for two days
